A new clinical entity, "local allergic rhinitis", was defined in 2010.

Local allergic rhinitis is defined as allergic rhinitis that occurs
with negative skin-prick and blood tests. It can occur with negative
intradermal allergy tests as well.

It implies that a huge number of people currently diagnosed with
nonallergic rhinitis, actually need to avoid allergens. The
researchers found local allergic rhinitis in about half of people
with "idiopathic rhinitis" - meaning rhinitis of unknown origin.

It doesn't seem to be controversial - I looked for papers taking issue
with it on Medline and didn't find any, nor allergists being skeptical
about it in blogs online.

Testing for it might typically be done at a university allergy department
rather than a local allergist, since it's complicated and probably
expensive.
